Some Veterans Affairs (VA) Hospitals prefer to setup an account with us prior to placing orders. To receive Cognistat's Tax ID information, you should fill out the request form below. Note that you must provide a valid e-mail address from a VA Hospital. Once we have confirmed the authenticity of the request, we will contact you.
